PATIenTS program, which aims to improve the trajectory of seniors in the transition between home and residential care in the RUISSS Universit\u00e9 Laval territory, has documented a list of improvements to facilitate the transition between home and Long-term care facilities (CHSLD).<\/p>\n\n\n<div class=\"block-accordion \">\n    <span class=\"block-accordion__label\">Accordion<\/span>\n (RUISSSUL)<\/p>\n\n\n<\/div>\n\n                    \n\n\n\n<h3 class=\"wp-block-heading\">Process for identifying improvements<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The team pooled and compared the following elements:<\/p>\n\n\n<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Recommendations identified in the grey literature;<\/li>\n\n\n\n<li>Best practices identified in the scientific literature;<\/li>\n\n\n\n<li>Recommendations and suggestions from interview analysis;<\/li>\n\n\n\n<li>Field observations;<\/li>\n\n\n\n<li>Current practices documented in interviews.<\/li>\n<\/ul>\n<\/div>\n\n\n<p class=\"wp-block-paragraph\">The improvements thus identified were divided into two categories and eight sub-categories.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">The improvements<\/h3>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link wp-element-button\" href=\"https:\/\/ssaquebec.wp.vortexdev.com\/wp-content\/uploads\/2024\/10\/Liste-des-ameliorations-de-Par-Aines.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Download the list (in French) or see in English below<\/a><\/div>\n<\/div>\n\n\n\n<p class=\"wp-block-paragraph\">The table presented in the document is classified by type of strategy:<\/p>\n\n\n<div class=\"wp-block-list\">\n<ol class=\"wp-block-list\">\n<li><strong>Strategies related to the structure of care and services<\/strong><div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Personalized and adapted living environment :<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Have a different architecture and layout from hospitals to reduce the feeling of being hospitalized and create a sense of familiarity.<\/li>\n\n\n\n<li>Facilitate the personalization of the room by giving access to it before moving in, and helping people who don&#8217;t have relatives to do this with them.<\/li>\n\n\n\n<li>Take people&#8217;s culture and beliefs into account when designing private and communal spaces<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Stability of human resources and continuity of care and services:<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Have stable teams to enable relational continuity<\/li>\n\n\n\n<li>Foster collaboration with residents and their families<\/li>\n\n\n\n<li>Hold weekly team meetings to follow up on files and case discussions.<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Management and administrative processes:<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Provide managers with tools to address the issues experienced by seniors and their loved ones during the transition to a CHSLD: training, online toolbox and forum, procedures guide, co-development workshops, etc.<\/li>\n\n\n\n<li>Integrate life history questionnaire into electronic file<\/li>\n\n\n\n<li>Adopt a continuous improvement approach in which facilities draw on evidence to make improvements<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Staff preparation and support:<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Ensure that staff understand all stages of the transition process and its effects on the individual.<\/li>\n\n\n\n<li>Allow staff time to help people settle in and acclimatize to their new environment<\/li>\n\n\n\n<li>Provide a practical guide outlining best practices to be adopted during the transition.<\/li>\n<\/ul>\n<\/div><\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li><strong>Strategies for quality of care and service processes<\/strong><div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Housing access mechanism:<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Establish a unique transition mechanism that respects resident preferences<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Admission and welcome procedures:<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Whenever possible, conduct a pre-move-in visit to familiarize the resident with the premises and the people, otherwise conduct a virtual visit.<\/li>\n\n\n\n<li>Draw up a life history in advance, so that it is available at the time of admission.<\/li>\n\n\n\n<li>Support the person and family in completing the life story<\/li>\n\n\n\n<li>At reception, staff introduce and welcome residents; they are welcoming, the center is visited, the experience must be positive for the person.<\/li>\n\n\n\n<li>Pair all residents with an attendant so that they have a resource person, someone who will act as an advocate for the facility.<\/li>\n\n\n\n<li>For the first week, identify other employees who can respond to residents when the attendant is not available.<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Life history :<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Add an introduction to the life history questionnaire to explain its relevance and use to the person and family.<\/li>\n\n\n\n<li>Complete the questionnaire before the person moves into the CHSLD.<\/li>\n\n\n\n<li>Complete the questionnaire with the person and family<\/li>\n\n\n\n<li>Have the life history completed by a person with training in helping relationships, so as to have the skills to discuss more sensitive or painful subjects.<\/li>\n\n\n\n<li>Make staff aware of the importance of the life story through training: how to fill it in and use it.<\/li>\n\n\n\n<li>Make the information available in the rooms<\/li>\n\n\n\n<li>Make greater use of the person&#8217;s life story<\/li>\n\n\n\n<li>Create a concise, easy-to-read format (summary)<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Aspects related to interpersonal relations :<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Develop tools\/interventions to improve communication and reception of indigenous people<\/li>\n\n\n\n<li>Use the person-centered approach and shared decision-making<\/li>\n\n\n\n<li>Respect residents&#8217; values, preferences, tastes and consent<\/li>\n\n\n\n<li>Facilitate the creation of new social relationships and participation in
