in support of clinical and organizational transformations in local care and services"},"content":{"rendered":"\n<p>In 2018, the Minist\u00e8re de la Sant\u00e9 et des Services sociaux (MSSS) launched the targeted grant program <em>La recherche ax\u00e9e sur le patient (RAP) en appui aux transformations cliniques et organisationnelles en soins et services de proximit\u00e9 int\u00e9gr\u00e9s<\/em> in partnership with the Unit\u00e9 de soutien SSA Qu\u00e9bec and the Fonds de recherche du Qu\u00e9bec &#8211; Sant\u00e9 (FRQS).<\/p>\n\n\n\n<p>This program is in line with Canada&#8217;s <a href=\"https:\/\/cihr-irsc.gc.ca\/e\/41204.html\">SPOR<\/a>, one of the flagship initiatives of the Canadian Institutes of Health Research (CIHR), of which the Unit\u00e9 de soutien SSA Qu\u00e9bec is an integral part as a provincial unit.<\/p>\n\n\n\n<figure class=\"wp-block-pullquote is-style-default has-small-font-size\"><blockquote><p>Transformational research in health and social services aims to ensure that the knowledge generated by research and best practices is more systematically focused on the user and integrated into clinical and organizational practices, as well as into the decision-making processes that govern care and service systems.<br><\/p><cite>Source: <a class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>4 MM<\/li>\n\n\n\n<li>Launched in July 2018 by the MSSS, the FRQ-S and the unit<\/li>\n\n\n\n<li>Pandemic extension to February 2023<\/li>\n\n\n\n<li>Objective to support the transformation of clinical and organizational practices into integrated local care and services, in support of the changes underway in the health and social services network.<\/li>\n<\/ul>\n<\/div>\n\n\n<h3 class=\"wp-block-heading\">The Unit&#8217;s contribution<\/h3>\n\n\n\n<p><br>As part of this program, the Unit provides teams with support and methodological expertise in relation to the requirements of patient-oriented research (POR).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Projects supported<\/h3>\n\n\n\n<p><br>Here are the projects supported by the Unit as part of the program.<\/p>\n\n\n<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li><strong>Effectively orienting users in the continuum of care and services by supporting collaborative practice involving users, GMF-U-Rs and professionals<\/strong> (CIUSSS de la Capitale-Nationale project team)<\/li>\n\n\n\n<li><strong>Evaluate the implementation of a pharmaceutical care trajectory between hospitals, GMFs and community pharmacies for seniors at risk of medication-related problems<\/strong> (CISSS de la Mont\u00e9r\u00e9gie-Centre project team)<\/li>\n\n\n\n<li><strong>Projet SOINS Diab\u00e8te: Structuring and organization of interprofessional care in partnership with diabetic patients followed in GMF-U <\/strong>(CISSS de Laval project team)<\/li>\n\n\n\n<li><strong>Support for the implementation of adapted interprofessional access in family medicine groups and university family medicine groups <\/strong>(CIUSS de l&#8217;Estrie-CHUS project team)<\/li>\n\n\n\n<li><strong>Training front-line trainers (F2PL)<\/strong> (CIUSSS du Saguenay-Lac-Saint-Jean project team)<\/li>\n<\/ul>\n<\/div>\n\n\n<p><a
