The ultimate aim of this program is to achieve the quintuple aim.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ultimately, the Unit seeks to support and facilitate the activation of program must meet the following criteria:<\/p>\n\n\n<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Selecting a life pathway :<div class=\"wp-block-list\">\n<ul class=\"wp-block-list\">\n<li>Complete and a priority for the RUISSS and its institutions<\/li>\n\n\n\n<li>Potentially transferable to all four RUISSSs<\/li>\n\n\n\n<li>Relevant to Ministry of Health and Social Services (aligned with its priorities)<\/li>\n\n\n\n<li>Characterized by the complete patient journey including trajectory mapping<\/li>\n<\/ul>\n<\/div><\/li>\n\n\n\n<li>Consideration of all of each RUISSS&#8217;s environments, including remote regions and indigenous territories, where relevant<\/li>\n\n\n\n<li>Project done in partnership with all stakeholders involved in research, care and management, patients and concerned communities<\/li>\n\n\n\n<li>Project leveraging the Unit&#8217;s LHS expertise to address the quintuple aim<\/li>\n\n\n\n<li>Team reporting based on the Unit&#8217;s performance measurement framework<\/li>\n\n\n\n<li>Achievement of the deliverables described in this program<\/li>\n\n\n\n<li>Relevant and realistic data acquisition and structuring strategy<\/li>\n\n\n\n<li>Use or deployment of existing technologies<\/li>\n\n\n\n<li>Consideration of the well-being of human resources, the feasibility and acceptability of the project by
